Aj ke\u010f ich morfol\u00f3gia m\u00f4\u017ee \u00a0by\u0165 variabiln\u00e1, \u00a0tvorba \u0161trukt\u00far podobn\u00fdch \u017e\u013eaz\u00e1m je v melanocytov\u00fdch n\u00e9voch zriedkav\u00fd \u00a0fenom\u00e9n. V odbornej literat\u00fare s\u00fa uveden\u00e9 preva\u017ene pr\u00edpady, ktor\u00e9 \u00a0sa d\u00e1vaj\u00fa \u00a0do \u00a0s\u00favislosti \u00a0so Spitzovej \u00a0n\u00e9vom(1-3). Pr\u00ed\u010dina vzniku t\u00fdchto morfologick\u00fdch zmien \u00a0aj ich klinick\u00fd v\u00fdznam nie s\u00fa jednozna\u010dne zn\u00e1me. V diagnostickej praxi patol\u00f3ga m\u00f4\u017ee \u00a0by\u0165 v\u00fdskyt pseudoglandul\u00e1rnych \u0161trukt\u00far v melanocytovej l\u00e9zii zav\u00e1dzaj\u00faci a v kone\u010dnom d\u00f4sledku s\u0165a\u017ei\u0165 spr\u00e1vnu diagnostiku chorobn\u00e9ho procesu.<\/p>\n<p>Uv\u00e1dzame pr\u00edpad \u00a0tohto \u00a0zvl\u00e1\u0161tneho variantu intraderm\u00e1lneho epiteloidn\u00e9ho n\u00e9vu \u00a0s tvorbou\u00a0\u00a0 pseudoglandul\u00e1rnych \u0161trukt\u00far u mladej \u00a0\u017eeny na ko\u017ei vonkaj\u0161\u00edch genit\u00e1li\u00ed.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Vlastn\u00fd <\/strong><strong>pr\u00edpad<\/strong><\/p>\n<p>35-ro\u010dn\u00e1 \u017eena \u00a0bola \u00a0vy\u0161etren\u00e1 s n\u00e1lezom sivo-ru\u017eov\u00e9ho ostro ohrani\u010den\u00e9ho bradavicovit\u00e9ho tumor\u00f3zneho \u00fatvaru ve\u013ekosti 8 mm na pravom ve\u013ekom pysku. Bola vykonan\u00e1 exc\u00edzna biopsia l\u00e9zie s n\u00e1slednou histologiz\u00e1ciou. L\u00e9ziu tvorilo v podko\u017e\u00ed lokalizovan\u00e9 jedno v\u00e4\u010d\u0161ie lo\u017eisko a po\u010detn\u00e9 men\u0161ie uzly preva\u017ene polygon\u00e1lnych a\u017e epiteloidn\u00fdch buniek <strong><em>(obr\u00e1zok 1)<\/em><\/strong>, \u010dasto s tvorbou centr\u00e1lnych, r\u00f4zne tvarovan\u00fdch cysticky pr\u00e1zdnych priestorov pripom\u00ednaj\u00facich \u017eliazky <strong><em>(obr\u00e1zok\u00a0 2)<\/em><\/strong>. Smerom do h\u013abky boli bunky usporiadan\u00e9 do tenk\u00fdch pruhov a\u017e jednotlivo roztr\u00fasen\u00e9. Bunky mali stredne objemn\u00fa eozinofiln\u00fa cytoplazmu s uniformn\u00fdmi jadrami \u00a0bez v\u00fdrazn\u00fdch jadierok, bez atypi\u00ed, bez mit\u00f3z. Ojedinele bunky vykazovali pigment\u00e1ciu. Na jednom mieste bola zachyten\u00e1 junk\u010dn\u00e1 aktivita.<\/p>\n<p>Imunohistochemick\u00e9 vy\u0161etrenie preuk\u00e1zalo expresiu prote\u00ednu S-100 a melanA <strong><em>(obr\u00e1zok 3-4) <\/em><\/strong>v epiteloidn\u00fdch bunk\u00e1ch l\u00e9zie. Tieto bunky boli negat\u00edvne na pancytokerat\u00edny (AE1\/3) a epitelov\u00fd \u00a0membr\u00e1nov\u00fd antig\u00e9n (EMA). Expresiu \u00a0CD34 vykazovali iba cievy podko\u017eia <strong><em>(obr\u00e1zok 5)<\/em><\/strong>. Prolifera\u010dn\u00e1 aktivita lezion\u00e1lnych buniek bola ve\u013emi n\u00edzka (Ki67 &lt; 1 %, <strong><em>obr\u00e1zok 6<\/em><\/strong>).<\/p>\n<p>Definit\u00edvna \u00a0diagn\u00f3za bola \u00a0stanoven\u00e1 ako\u00a0 intraderm\u00e1lny epiteloidn\u00fd n\u00e9vus \u00a0so pseudoglandul\u00e1rnou morfol\u00f3giou.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Diskusia<\/strong><\/p>\n<p>Glandul\u00e1rna diferenci\u00e1cia je typickou \u00a0\u010drtou \u00a0najm\u00e4 epitelov\u00fdch n\u00e1dorov. Melanocyty nemaj\u00fa schopnos\u0165 tvori\u0165 prav\u00e9 \u017e\u013eazov\u00e9 \u0161trukt\u00fary. V\u00fdskyt pseudoglandul\u00e1rnych zmien je v melanocytov\u00fdch l\u00e9zi\u00e1ch zriedkavou \u010drtou. Tvorbu tubulov a cystick\u00fdch priestorov v epiteloidnom derm\u00e1lnom n\u00e9ve ako\u00a0 prv\u00ed op\u00edsali \u00a0Burg a spol.(1), ktor\u00ed t\u00fato \u00a0l\u00e9ziu ozna\u010dili \u00a0ako \u201etubul\u00e1rny epiteloidn\u00fd n\u00e9vus\u201c, a pova\u017eovali ho za variant Spitzovej n\u00e9vu. Nesk\u00f4r \u00a0boli op\u00edsan\u00e9 \u010fal\u0161ie pr\u00edpady s v\u00fdskytom pseudoglandul\u00e1rnych zmien. Pr\u00e1ve Spitzovej n\u00e9vus sa\u00a0v odbornej literat\u00fare naj\u010dastej\u0161ie spom\u00edna v s\u00favislosti so vznikom pseudoglandul\u00e1rnych form\u00e1ci\u00ed(4-6). Vzh\u013eadom \u00a0na preva\u017ene epiteloidn\u00fa morfol\u00f3giu a\u00a0 v\u00fdskyt \u00a0u mlad\u00e9ho dospel\u00e9ho \u010dloveka \u00a0niektor\u00e9 nazna\u010den\u00e9 spitzoidn\u00e9 rysy vykazuje aj nami \u00a0prezentovan\u00e1 l\u00e9zia. \u00a0Asoci\u00e1cia Spitzovej n\u00e9vu so pseudoglandul\u00e1rnou\u00a0 morfol\u00f3giou v\u0161ak \u00a0nie je v\u00fdhradn\u00e1. Soyer \u00a0a spol.(3) \u00a0pouk\u00e1zali, \u00a0\u017ee m\u00f4\u017ee \u00a0by\u0165 pozorovan\u00e1 v ove\u013ea \u0161ir\u0161om spektre melanocytov\u00fdch l\u00e9zi\u00ed vr\u00e1tane dysplastick\u00fdch \u00a0(Clarkov\u00fdch) \u00a0n\u00e9vov. Pseudoglandul\u00e1rny variant \u00a0je zn\u00e1mym a dobre \u00a0op\u00edsan\u00fdm, aj ke\u010f zriedkav\u00fdm variantom mal\u00edgneho melan\u00f3mu(3,7).<\/p>\n<p>Histologicky \u00a0je \u00a0tvorba \u00a0tubulov \u00a0a \u00a0cystick\u00fdch \u00a0priestorov v melanocytov\u00fdch l\u00e9zi\u00e1ch sk\u00f4r lo\u017eiskovou fok\u00e1lnou zmenou. Len niektor\u00ed autori \u00a0ich opisuj\u00fa \u00a0ako dominantn\u00fa \u010drtu(1,2), \u010dasto a\u017e s tvorbou mikrocystick\u00fdch \u0161trukt\u00far. Aj v nami predlo\u017eenom pr\u00edpade bol v\u00fdskyt pseudoglandul\u00e1rnych zmien poprednou \u010drtou \u00a0l\u00e9zie tvorenej \u00a0preva\u017ene epiteloidn\u00fdmi bunkami, bez v\u00fdraznej\u0161\u00edch atypi\u00ed alebo \u00a0mit\u00f3z.<\/p>\n<p>Nie je zn\u00e1ma asoci\u00e1cia medzi v\u00fdskytom pseudoglandul\u00e1rnych zmien \u00a0a vekom, resp. \u00a0pohlav\u00edm \u00a0pacientov. V literat\u00fare op\u00edsan\u00e9 pr\u00edpady maj\u00fa \u0161irok\u00fd vekov\u00fd rozptyl. Tento \u00a0n\u00e1lez nes\u00favis\u00ed ani s lokalitou v\u00fdskytu l\u00e9zie. S\u00fa op\u00edsan\u00e9 pr\u00edpady na horn\u00fdch(2,8) aj doln\u00fdch kon\u010datin\u00e1ch(1). V\u00fdskyt v oblasti vonkaj\u0161ieho genit\u00e1lu z literat\u00fary zn\u00e1my \u00a0nie je.<\/p>\n<p>Predpoklad\u00e1 sa, \u017ee \u00a0v melanocytov\u00fdch l\u00e9zi\u00e1ch \u00a0je \u00a0vznik pseudoglandul\u00e1rnych form\u00e1ci\u00ed \u00a0sp\u00f4soben\u00fd najpravdepodobnej\u0161ie retrak\u010dn\u00fdm artefaktom ako \u00a0n\u00e1sledok fix\u00e1cie \u00a0tkaniva formaldehydom. Zd\u00e1 \u00a0sa, \u017ee ur\u010dit\u00e9 \u00a0typy n\u00e9vov s\u00fa n\u00e1chyln\u00e9 na tvorbu\u00a0 ur\u010dit\u00fdch \u00a0artefaktov. Op\u00edsan\u00fd \u00a0je v\u00fdskyt pseudovaskul\u00e1rnych \u0161trukt\u00far preva\u017ene v polypoidn\u00fdch a veruk\u00f3znych n\u00e9voch(9,10). Ziemer \u00a0a spol.(6) \u00a0pozorovali v\u00fdskyt r\u00f4znych druhov \u00a0artefaktov podmienen\u00fdch fix\u00e1ciou \u00a0vo vy\u0161e \u00a080 \u00a0% sk\u00faman\u00fdch Spitzovej \u00a0n\u00e9vov. \u00a0\u017d\u013eazov\u00e9 \u0161trukt\u00fary pripom\u00ednaj\u00face \u201epr\u00e1zdne priestory\u201c sa typicky vyskytovali, podobne ako v na\u0161om pr\u00edpade, v n\u00e9voch tvoren\u00fdch preva\u017ene alebo aspo\u0148 \u010diasto\u010dne z epiteloidn\u00fdch buniek.<\/p>\n<p>Vznik tubul\u00e1rnych \u0161trukt\u00far v melanocytov\u00fdch l\u00e9zi\u00e1ch m\u00f4\u017ee by\u0165 sp\u00f4soben\u00fd aj apopt\u00f3zou centr\u00e1lnych buniek v hniezdach melanocytov(1). Sledovan\u00edm apoptotick\u00fdch zmien \u00a0v s\u00fabore Spitzovej n\u00e9vov v\u0161ak Ziemer a spol.(6) t\u00fato mo\u017enos\u0165 sk\u00f4r vylu\u010duj\u00fa. Autori sa s\u00fastredili pr\u00e1ve \u00a0na mo\u017en\u00fa \u00falohu \u00a0apopt\u00f3zy a vo svojej \u00a0\u0161t\u00fadii nezistili \u00a0pr\u00edtomnos\u0165 apoptotick\u00fdch buniek v oblasti pseudoglandul\u00e1rnych \u0161trukt\u00far a imunohistochemicky ani geneticky nepreuk\u00e1zali zv\u00fd\u0161en\u00fa aktiv\u00e1ciu apopt\u00f3zy pri porovn\u00e1van\u00ed melanocytov\u00fdch l\u00e9zi\u00ed so pseudoglandul\u00e1rnymi \u0161trukt\u00farami a bez nich. Pr\u00edtomnos\u0165 apoptotick\u00fdch fig\u00far sme v nami op\u00edsanom pr\u00edpade nepozorovali ani my.<\/p>\n<p>Pod\u013ea \u00a0\u010fal\u0161ej \u00a0te\u00f3rie \u00a0by mohla \u00a0by\u0165 tvorba \u00a0\u00fatvarov \u00a0podobn\u00fdch \u017e\u013eaz\u00e1m prejavom diferenci\u00e1cie smerom k neur\u00e1lnym \u0161trukt\u00faram. Kantrow a spol.(5) op\u00edsali\u00a0 Spitzovej n\u00e9vus \u00a0s tvorbou \u00a0\u0161trukt\u00far pripom\u00ednaj\u00facich Homerove-Wrightove rozety. In\u00fd pr\u00edpad \u00a0Spitzovej \u00a0n\u00e9vu \u00a0s tvorbou \u00a0rozetovit\u00fdch form\u00e1ci\u00ed, pripom\u00ednaj\u00facich sk\u00f4r \u201eprav\u00e9 epend\u00fdmov\u00e9 rozety\u201c, zdokumentoval Fujita spol.(8). \u00a0Ide v\u0161ak o \u0161pecifick\u00fd typ zmien, ktor\u00fd sa morfologicky odli\u0161uje \u00a0od typick\u00fdch pseudoglandul\u00e1rnych \u0161trukt\u00far.<\/p>\n<p>Pseudoglandul\u00e1rna morfol\u00f3gia m\u00f4\u017ee \u00a0predstavova\u0165 za ur\u010dit\u00fdch okolnost\u00ed diagnostick\u00fd probl\u00e9m pre hodnotiaceho patol\u00f3ga. Prvotn\u00e9 \u00a0diagnostick\u00e9 \u00favahy v nami \u00a0prezentovanom pr\u00edpade zah\u0155\u0148ali \u00a0okrem \u201enezvy\u010dajnej\u201c melanocyt\u00e1rnej l\u00e9zie, n\u00e1dor \u00a0z ko\u017en\u00fdch \u00a0adnexov, metast\u00e1zu adenokarcin\u00f3mu alebo l\u00e9ziu vaskul\u00e1rneho p\u00f4vodu. Imunohistochemick\u00e9 farbenie v\u0161ak jednozna\u010dne preuk\u00e1zalo pozitivitu buniek na prote\u00edn S-100 a melanA, pri s\u00fa\u010dasnej negativite pancytokerat\u00ednov AE1\/3, EMA a vaskul\u00e1rneho markera CD34, a tak umo\u017enilo vyl\u00fa\u010di\u0165 ostatn\u00e9 diferenci\u00e1lnodiagnostick\u00e9 mo\u017enosti. Blandn\u00e1 histol\u00f3gia, absencia atypi\u00ed spolu s n\u00edzkou \u00a0prolifera\u010dnou aktivitou \u00a0(Ki67 &lt; 1 %) podporili \u00a0diagn\u00f3zu ben\u00edgnej melanocytovej l\u00e9zie.<\/p>\n<p>Klinick\u00fd v\u00fdznam n\u00e1lezu pseudoglandul\u00e1rnych zmien v melanocytov\u00fdch l\u00e9zi\u00e1ch \u00a0nebol doposia\u013e objasnen\u00fd. V\u00fdznam pre diagnostick\u00fa prax tkvie hlavne v zamedzen\u00ed vzniku diagnostick\u00e9ho\u00a0 omylu pri hodnoten\u00ed be\u017en\u00fdch jednotiek s nezvy\u010dajn\u00fdmi morfologick\u00fdmi zmenami.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Z\u00e1ver<\/strong><\/p>\n<p>V\u00fdskyt pseudoglandul\u00e1rnych \u0161trukt\u00far v melanocytov\u00fdch l\u00e9zi\u00e1ch \u00a0je zriedkavou zmenou, treba \u00a0v\u0161ak \u00a0na t\u00fato mo\u017enos\u0165 pam\u00e4ta\u0165 predov\u0161etk\u00fdm v pr\u00edpade rozsiahlej tvorby tubul\u00e1rnych alebo cystick\u00fdch \u00fatvarov, ke\u010f nie s\u00fa zachovan\u00e9 takmer \u017eiadne typick\u00e9 hniezda n\u00e9vov\u00fdch \u00a0buniek. Even until today, causes of their development as well as their potential Clinical relevance have not been fully clarified. They have been described particularly in the case of Spitz nevus, however, they can be observed practically in any melanocytic lesion. At present, it is expected that development of pseudoglandular formations rather represents a retraction artefact related to fixing and processing of the tissue.
