Uroliti\u00e1za bola zn\u00e1ma choroba u\u017e v staroveku, kamene mo\u010dov\u00e9ho mech\u00fara boli \u010dast\u00e9 najm\u00e4 u mlad\u00fdch chlapcov a star\u00fdch mu\u017eov, boli zaznamenan\u00e9 pokusy o konzervat\u00edvnu lie\u010dbu a\u00a0dokonca o rozp\u00fa\u0161\u0165anie kame\u0148ov. Ranhoji\u010di zvl\u00e1dali oper\u00e1cie bez nark\u00f3zy a technick\u00e9 podrobnosti sa udr\u017eovali ako rodinn\u00e9 tajomstvo.<\/p>\n<p>Konkrement je zlo\u017een\u00fd z\u00a098% z\u00a0kry\u0161talick\u00e9ho materi\u00e1lu a\u00a0pribli\u017ene 2% tvor\u00ed organick\u00e1 hmota zlo\u017een\u00e1 z\u00a0prote\u00ednov, glykoprote\u00ednov, cukrov, ktor\u00e9 sp\u00e1jaj\u00fa kry\u0161t\u00e1ly do v\u00fdsledn\u00fdch tvarov konkrementov. V\u00fdnimku tvoria infek\u010dn\u00e9 mo\u010dov\u00e9 konkrementy (struvit), kde polysacharidy produkovan\u00e9 bakt\u00e9riami tvoria nadpolovi\u010dn\u00fa \u010das\u0165 kame\u0148a.<\/p>\n<p>MK s\u00e1m o\u00a0sebe nie je chorobou, ale je n\u00e1sledkom celkov\u00e9ho ochorenia na podklade metabolickej poruchy, infekcie mo\u010dov\u00fdch ciest alebo morfologick\u00fdch a\u00a0funk\u010dn\u00fdch zmien uropoetick\u00e9ho syst\u00e9mu. Incidencia uroliti\u00e1zy je v\u00a0celosvetovom meradle vysok\u00e1 (cca 10%) (5), \u00a0narast\u00e1 po\u010det recid\u00edv, pri\u010dom prevencia \u2013 metafylaxia m\u00f4\u017ee zn\u00ed\u017ei\u0165\u00a0 v\u00fdskyt recid\u00edv zo 70% na 10-15% (2, 3). Cenn\u00fa inform\u00e1ciu v\u00a0terapii a\u00a0prevencii recid\u00edv poskytuje d\u00f4sledn\u00e9 zhodnotenie typu liti\u00e1zy pod\u013ea chemick\u00e9ho zlo\u017eenia konkrementu a\u00a0pod\u013ea fyzik\u00e1lnej anal\u00fdzy.<\/p>\n<p>Vznik MK je v\u00fdsledkom vz\u00e1jomn\u00e9ho p\u00f4sobenia ochrann\u00fdch a\u00a0rizikov\u00fdch faktorov. S\u00fa zn\u00e1me kauz\u00e1lne faktory uroliti\u00e1zy, ktor\u00fdch vz\u00e1jomn\u00fdm p\u00f4soben\u00edm sa m\u00f4\u017ee MK vytvori\u0165.<\/p>\n<p><strong>Kauz\u00e1lne faktory uroliti\u00e1zy <\/strong><\/p>\n<ol>\n<li><strong> Pres\u00fdtenie mo\u010du (supersatur\u00e1cia) litog\u00e9nnymi l\u00e1tkami <\/strong>pri metabolickej poruche, nadmernom prijme potravy, poruch\u00e1ch gastrointestinalneho traktu (GIT), u\u017eivan\u00ed niektor\u00fdch liekov a pod.<\/li>\n<li><strong>Zmeny pH mo\u010du, <\/strong> pri ren\u00e1lnej tubul\u00e1rnej acid\u00f3ze (RTA), prim\u00e1rnej dne, infekcii bakt\u00e9riami \u0161tiepiacimi mo\u010dovinu, pri poruch\u00e1ch GIT-u a pod.<\/li>\n<li><strong> Nedostatok inhib\u00edtorov <\/strong>kry\u0161taliz\u00e1cie a agreg\u00e1cie.<\/li>\n<li><strong> Ob\u0161truk\u010dn\u00e9 uropatie a morfologick\u00e9 zmeny obli\u010diek, <\/strong>napr. dre\u0148ov\u00e1 cyst\u00f3za, anom\u00e1lie mo\u010dov\u00fdch ciest, funk\u010dn\u00e9 poruchy a pod.<\/li>\n<li><strong> Cudzie teleso <\/strong>v mo\u010dov\u00fdch cest\u00e1ch.<\/li>\n<\/ol>\n<p>U\u00a0v\u00e4\u010d\u0161iny\u00a0 chor\u00fdch zist\u00edme nieko\u013eko rizikov\u00fdch faktorov, \u010do potvrdzuje multifaktori\u00e1lnu etiol\u00f3giu uroliti\u00e1zy. Najd\u00f4le\u017eitej\u0161ie rizikov\u00e9 faktory predstavuj\u00fa metabolick\u00e9 poruchy, ktor\u00e9 sa vyskytuj\u00fa a\u017e u\u00a090% chor\u00fdch s\u00a0liti\u00e1zou (4).<\/p>\n<p><strong>Typy mo\u010dov\u00fdch konkrementov<\/strong><\/p>\n<ol>\n<li><strong> Z\u00e1kladn\u00e1 skupina MK<\/strong><\/li>\n<\/ol>\n<p><strong>Kalciumoxal\u00e1tov\u00e9\u00a0 ( <\/strong>weddellit, whewellit),<strong> kalciumfosf\u00e1tov\u00e9<\/strong> (apatit, brushit), <strong>kyselina mo\u010dov\u00e1<\/strong> (dihydr\u00e1t kyseliny mo\u010dovej), <strong>struvit <\/strong>a<strong>\u00a0cyst\u00edn. <\/strong><\/p>\n<ol start=\"2\">\n<li><strong> Ojedinel\u00e9 MK<\/strong><\/li>\n<\/ol>\n<p>Mo\u010dan sodn\u00fd, mo\u010dan am\u00f3nny, beta-fosfore\u010dnan v\u00e1penat\u00fd (whitlockit), hydrog\u00e9nfosfore\u010dnan v\u00e1penat\u00fd (monetit), xantin, DHA \u2013 2,8- dihydroxyadenin.<\/p>\n<ol start=\"3\">\n<li><strong> Liekov\u00e9 MK a\u00a0ich metabolity<\/strong> &#8211; oxytetracykl\u00edn, ampicil\u00edn, sulfonamidy, chloramphenikol, dezurol , crivixan&#8230;&#8230;<\/li>\n<li><strong> Artefaky &#8211; <\/strong>alfa kreme\u0148, kalcit, s\u00e1drovec, tehla, paraf\u00edn, omietka, pieskovec, granit, keramzit, semienka rastl\u00edn&#8230;..<\/li>\n<\/ol>\n<p>V\u0161etky MK nezaraden\u00e9 do z\u00e1kladnej skupiny vy\u017eaduj\u00fa \u0161peci\u00e1lne analytick\u00e9 postupy d\u00f4kazu jednotliv\u00fdch komponentov.<\/p>\n<p><strong>Mokr\u00e1 cesta anal\u00fdzy<\/strong><\/p>\n<p>Predstavuje \u00a0semikvantitat\u00edvnu,\u00a0 rutinn\u00fa met\u00f3du na identifik\u00e1ciu d\u00f4le\u017eit\u00fdch komponentov MK spont\u00e1nne vymo\u010den\u00fdch\u00a0 ako aj vyoperovan\u00fdch. \u00a0Je ekonomick\u00e1 a\u00a0r\u00fdchla. Pou\u017eit\u00edm setu Urinary calculi (DiaSys) s\u00fa dok\u00e1zate\u013en\u00e9 oxal\u00e1ty, fosf\u00e1ty, hor\u010d\u00edk, amoniak, kyselina mo\u010dov\u00e1, cyst\u00edn a to farebn\u00fdmi reakciami, v\u00e1pnik titra\u010dne a\u00a0karbon\u00e1ty pridan\u00edm HCl. Zlo\u017eenie MK sa vypo\u010d\u00edta pomocou prav\u00edtka, ktor\u00e9 ja s\u00fa\u010das\u0165ou setu.<\/p>\n<p>Sp\u00f4sob anal\u00fdzy poskytuje len hrub\u00fa orient\u00e1ciu v\u00a0zlo\u017een\u00ed MK patriacich do z\u00e1kladnej skupiny. Spr\u00e1vnos\u0165 a\u00a0spo\u013eahlivos\u0165 v\u00fdsledkov chemickej anal\u00fdzy mokrou cestou je potrebn\u00e9 potvrdi\u0165 pou\u017eit\u00edm inej met\u00f3dy, v\u00a0na\u0161om laborat\u00f3riu \u00a0polariza\u010dnej mikroskopie.<\/p>\n<p>Smernice pre uroliti\u00e1zu Eur\u00f3pskej asoci\u00e1cie pre urol\u00f3giu (EAU), neodpor\u00fa\u010daj\u00fa mokr\u00fa cestu anal\u00fdzy MK , dokonca ju pova\u017euj\u00fa za neakcepovat\u013en\u00fa (1).<\/p>\n<p>V\u00a0na\u0161ich podmienkach set pom\u00f4\u017ee pri identifik\u00e1cii oxal\u00e1tov MK nezn\u00e1meho, zmie\u0161an\u00e9ho zlo\u017eenia.<\/p>\n<p><strong><br \/>\nPolariza\u010dn\u00e1 mikroskopia<\/strong><\/p>\n<p>Je to mikroskopick\u00e1 met\u00f3da, ktor\u00e1 vyu\u017e\u00edva optick\u00e9 vlastnosti kry\u0161t\u00e1lov v prech\u00e1dzaj\u00facom svetle. Polariz\u00e1torom prech\u00e1dza svetlo a men\u00ed sa na line\u00e1rne polarizovan\u00e9. Po prechode anizotropnou vzorkou (kry\u0161t\u00e1lom) sa rozdel\u00ed na dva l\u00fa\u010de &#8211; riadny a mimoriadny, \u0161iria sa r\u00f4znou r\u00fdchlos\u0165ou a kmitaj\u00fa v r\u00f4znych rovin\u00e1ch. analyz\u00e1tor zlo\u017e\u00ed obidva l\u00fa\u010de do rovnakej roviny kmitania a ich f\u00e1zov\u00fd posun sa prejav\u00ed vznikom interferen\u010dn\u00fdch farieb. Pri amorfn\u00fdch zlo\u017ek\u00e1ch (apatit, karbon\u00e1tapatit) nevznik\u00e1 f\u00e1zov\u00fd posun a ani interferen\u010dn\u00e9 farby, obrazy MK\u00a0 predstavuj\u00fa bezfarebn\u00e9 \u00fatvary.<\/p>\n<p>Polariza\u010dn\u00e1 mikroskopia je citliv\u00e1 met\u00f3da, ktor\u00e1 n\u00e1m umo\u017e\u0148uje stanovi\u0165 aj ve\u013emi mal\u00e9 mno\u017estvo analytu. M\u00e1 v\u0161ak obmedzenia, ktor\u00e9 sa t\u00fdkaj\u00fa optick\u00fdch vlastnost\u00ed materi\u00e1lu. Nedaj\u00fa sa ur\u010di\u0165 jemnozrnn\u00e9 a\u017e amorfn\u00e9 materi\u00e1ly, nezn\u00e1me vzorky a\u00a0semikvantitat\u00edvne stanovenie je mo\u017en\u00e9 len na z\u00e1klade odhadu objemov\u00fdch percent, \u010do m\u00f4\u017ee by\u0165 rozdielne od hmotnostn\u00fdch percent stanoven\u00fdch infra\u010dervenou spektroskopiou. Podlieha vo ve\u013ekej miere subjekt\u00edvnej chybe, je \u0165a\u017eko \u0161tandardizovate\u013en\u00e1 a\u00a0doba z\u00e1cviku je pomerne dlh\u00e1. Spojenie s\u00a0I\u010c v\u0161etky tieto nedostatky eliminuje.<\/p>\n<p><strong>Infra\u010derven\u00e1 spetroskopia.<\/strong><\/p>\n<p>Sk\u00fama absorpciu infra\u010derven\u00e9ho \u017eiarenia molekulami vzorky. Poskytuje inform\u00e1cie o pr\u00edtomn\u00fdch funk\u010dn\u00fdch skupin\u00e1ch a o molekulovej \u0161trukt\u00fare l\u00e1tky a\u00a0sl\u00fa\u017ei k\u00a0ich kvantitat\u00edvnemu stanoveniu. V\u00a0I\u010c oblasti je akt\u00edvna v\u00e4\u010d\u0161ina molek\u00fal. Absorp\u010dn\u00e9 spektrum je charakteristick\u00e9 pre dan\u00fa l\u00e1tku, prakticky neexistuj\u00fa dve rovnak\u00e9 spektr\u00e1 pre r\u00f4zne l\u00e1tky s\u00a0v\u00fdnimkou optick\u00fdch izom\u00e9rov. Vysok\u00e1 incidencia, recidivita, nepr\u00edjemn\u00e9 ak\u00fatne prejavy a\u00a0mo\u017en\u00e9 v\u00e1\u017ene n\u00e1sledky s\u00fa nepochybne d\u00f4vodom pre \u0161pecifick\u00fa metafylaxiu uroliti\u00e1zy, predov\u0161etk\u00fdm medikament\u00f3znu, a\u00a0to najlep\u0161ie u\u017e u\u00a0chor\u00fdch s\u00a0prvou pr\u00edhodou uroliti\u00e1zy (3).
