Rickettsie t\u00fdfusovej skupiny R. typhi a R. prowazekii, p\u00f4vodca historicky zn\u00e1meho \u0161kvrnit\u00e9ho t\u00fdfusu, sa pren\u00e1\u0161aj\u00fa v\u0161ami a blchami. Rickettsie zo skupiny purpurov\u00fdch hor\u00fa\u010dok (SFGR) s\u00fa pren\u00e1\u0161an\u00e9 najm\u00e4 klie\u0161\u0165ami. Vstupnou br\u00e1nou je teda ko\u017ea, odkia\u013e sa rickettsie roz\u0161iruj\u00fa krvn\u00fdm obehom a infikuj\u00fa endotel, niekedy aj vaskul\u00e1rny syst\u00e9m hladk\u00e9ho svalstva(1). Na Slovensku s\u00fa rickettsie pren\u00e1\u0161an\u00e9 najm\u00e4 klie\u0161\u0165ami Ixodes ricinus, Dermacentor reticulatus a D. marginatus (2). V r. 2011 bolo zriaden\u00e9 N\u00e1rodn\u00e9 referen\u010dn\u00e9 centrum pre surveillance a laborat\u00f3rnu diagnostiku rickettsi\u00f3z pri Region\u00e1lnom \u00farade verejn\u00e9ho zdravotn\u00edctva v Banskej Bystrici, ktor\u00e9ho hlavn\u00fdm poslan\u00edm je uskuto\u010d\u0148ova\u0165 s\u00e9rologick\u00fa diagnostiku infekci\u00ed sp\u00f4soben\u00fdch rickettsiami a Coxiella burnetii.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Materi\u00e1l a met\u00f3dy<\/strong><\/p>\n<p>S\u00e9ra pacientov sme obdr\u017eali z ambulanci\u00ed o\u0161etruj\u00facich lek\u00e1rov po celom Slovensku. Na detekciu IgG protil\u00e1tok v ELISA a IFA (pri \u0161edej z\u00f3ne ELISA) testoch sme pou\u017eili antig\u00e9n Rickettsia akari (RaAg, R) pripraven\u00fd na na\u0161om oddelen\u00ed a komer\u010dn\u00e9 ELISA kity Spotted Fever Rickettsia IgG (Fuller Lab, F) a Rickettsia conorii ELISA IgG\/IgM (Vircell, V). Inform\u00e1cie o pacientovi (pohlavie, vek, bydlisko, diagn\u00f3za, klinick\u00e9 pr\u00edznaky a zaklie\u0161tenie) uveden\u00e9 na \u017eiadank\u00e1ch sme vyu\u017eili pre \u0161tatistick\u00e9 vyhodnotenie v\u00fdznamnost\u00ed (p \u2264 0,05) v priamej s\u00favislosti s v\u00fdsledkami testov<br \/>\n(http:\/\/www.openepi.com\/RbyC\/RbyC.htm). N\u00e1sledne sme zhodnotili citlivos\u0165 a \u0161pecifickos\u0165 (https:\/\/www.medcalc. org\/calc\/diagnostic_test.php).<\/p>\n<p>&nbsp;<\/p>\n<p><strong>V\u00fdsledky<br \/>\n<\/strong><br \/>\nPr\u00edtomnos\u0165 IgG protil\u00e1tok vo\u010di SFGR bola potvrden\u00e1 oboma komer\u010dn\u00fdmi ELISA kitmi v 9 pr\u00edpadoch. RaAg poskytol pozit\u00edvny v\u00fdsledok pri 10 s\u00e9rach, z ktor\u00fdch 8 bolo v zhode s komer\u010dn\u00fdmi testami. Len 1 s\u00e9rum nebolo rozpoznan\u00e9 ako pozit\u00edvne. N\u00e1\u0161 test nazna\u010dil \u0161ed\u00fa z\u00f3nu, a preto pozitivita bola potvrden\u00e1 a\u017e s IFA. V\u0161etky tri testy vyk\u00e1zali zhodu aj v nepr\u00edtomnos\u0165 protil\u00e1tok pri 88 s\u00e9rach. V\u00fdsledky sa l\u00ed\u0161ili len v 12 pr\u00edpadoch, kde jeden z testov nazna\u010dil pozit\u00edvny v\u00fdsledok a pri 30-tich \u0161ed\u00fa z\u00f3nu. N\u00e1sledne bola vypo\u010d\u00edtan\u00e1 citlivos\u0165 (pre R 92,9 %, F 100% a V 78,6 %) a \u0161pecifickos\u0165 (pre R 76,8 %, F 95,2 % a V 98,4 %) jednotliv\u00fdch testov. S\u00e9roprevalencia bola signifikantne vy\u0161\u0161ia u pacientov nad 41 rokov (11,9 %), \u017eij\u00facich v meste (8,6 %; p = 0,041) a u pacientov s pozit\u00edvnou symptomatol\u00f3giou pre rickettsi\u00f3zy (12,3 %; p = 0,006). Rozdiely v s\u00e9roprevalencii pre opa\u010dn\u00e9 pohlavia (\u017eena: 43,7 %, mu\u017ei: 59,7 %) neboli \u0161tatisticky v\u00fdznamn\u00e9 (p = 0,364).<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Diskusia<br \/>\n<\/strong><br \/>\nV\u0161etk\u00fdmi pou\u017eit\u00fdmi ELISA testami sme potvrdili pr\u00edtomnos\u0165 IgG protil\u00e1tok vo\u010di SFGR u 5,8 % pacientov zo Slovenska. Na Slovensku by mohlo \u00eds\u0165 o infekcie sp\u00f4soben\u00e9 R. helvetica, R. monacensis, R. slovaca alebo R. raoultii. Na porovnanie v Taliansku bolo detegovan\u00fdch pribli\u017ene 1000 pr\u00edpadov infikovan\u00fdch Rickettsia conorii(3) a v Portugalsku 9 pr\u00edpadov ro\u010dne na 100,000 obyvate\u013eov(4). V Belgicku, \u0160vaj\u010diarsku, \u010di vo Franc\u00fazsku boli diagnostikovan\u00e9 infekcie s R. conorii priemerne u 50 pacientov ro\u010dne na 100,000 obyvate\u013eov(5).<br \/>\nNa\u0161e v\u00fdsledky potvrdzuj\u00fa cirkul\u00e1ciu Rickettsia spp. na Slovensku. Poukazuj\u00fa t\u00fdm aj na skuto\u010dnos\u0165, \u017ee diagnostike t\u00fdchto infek\u010dn\u00fdch ochoren\u00ed je potrebn\u00e9 venova\u0165 pozornos\u0165, a to najm\u00e4<br \/>\nv pr\u00edpadoch, ke\u010f pr\u00edznaky ochorenia nazna\u010duj\u00fa p\u00f4vod infekcie.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Z\u00e1ver<\/strong><br \/>\nELISA test na diagnostiku rickettsiov\u00fdch infekci\u00ed zalo\u017een\u00fd na antig\u00e9ne R. akari vyvinutom na BMC SAV poskytuje zodpovedn\u00e9 v\u00fdsledky, ktor\u00e9 s\u00fa zhodn\u00e9 s komer\u010dn\u00fdmi testami, a to najm\u00e4 v pr\u00edpade ak sa v\u00fdsledky v \u0161edej z\u00f3ne potvrdia s IFA. Using this antigen, we examined 139 patient sera for the presence of IgG antibodies to rickettsiae and compared to the results of two commercial ELISA kits from Fuller Lab and Vircell.
