Parazit, medzihostite\u013e \u2212 sladkovodn\u00fd ulitn\u00edk Bulinus globous a kone\u010dn\u00fd hostite\u013e tvoria spolo\u010denstvo v dobre definovanom ekosyst\u00e9me, v ktorom sa parazit reprodukuje a \u0161\u00edri. Snaha o kontrolu schistosom\u00f3zy v tejto oblasti spo\u010d\u00edva v hromadnom pod\u00e1van\u00ed jednej d\u00e1vky praziquantelu \u0161kolopovinn\u00fdm de\u0165om raz ro\u010dne. Cie\u013eom na\u0161ej pr\u00e1ce bolo monitorovanie stavu kontamin\u00e1cie vybran\u00fdch vyu\u017e\u00edvan\u00fdch vodn\u00fdch zdrojov ulitn\u00edkmi druhu B. globosus, preto\u017ee preru\u0161enie transmisie v\u00fdvojov\u00fdch \u0161t\u00e1di\u00ed parazita (cerk\u00e1ri\u00ed) z medzihostite\u013ea na \u010dloveka by mohlo ma\u0165 v mana\u017emente tohto ochorenia z\u00e1sadn\u00fd v\u00fdznam.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Materi\u00e1l a metodika<br \/>\n<\/strong><br \/>\nV lokalite Mwachiga, Mwaluphamba, Bilashaka a Mbuwani (Kwale county, Ke\u0148a) sme sledovali premorenie vybran\u00fdch vodn\u00fdch zdrojov infikovan\u00fdmi ulitn\u00edkmi druhu B. globosus. Na detekciu pr\u00edtomnosti patentn\u00e9ho \u0161t\u00e1dia parazita sme ulitn\u00edky B. globosus podrobili testu uvo\u013e\u0148ovania cerk\u00e1ri\u00ed. Na neskor\u0161ie stanovenie pr\u00edtomnosti prepatentn\u00e9ho parazit\u00e1rneho \u0161t\u00e1dia pomocou dvoch po sebe nasleduj\u00facich polymer\u00e1zov\u00fdch re\u0165azov\u00fdch reakci\u00ed (PCR) sme hemolymfu ulitn\u00edkov zachytili na FTA mikrokarty (obr\u00e1zok 1). Pre DraI a Sh110\/SmSl PCR sme pou\u017eili primery pod\u013ea Amarir et al., 2014(1).<\/p>\n<p>&nbsp;<\/p>\n<p><strong>V\u00fdsledky<\/strong><\/p>\n<p>Medzihostite\u013e infekcie \u2013 B. globosus \u2013 sa na\u0161iel v 10 zo 16 sledovan\u00fdch vodn\u00fdch zdrojov. Test na uvo\u013e\u0148ovanie cerk\u00e1ri\u00ed bol pozit\u00edvny v 2 vy\u0161etrovan\u00fdch lokalit\u00e1ch, a to na rieke Pengo a Tsanganyiko. Genetick\u00fd materi\u00e1l (hemolymfa) 68 slim\u00e1kov B. globosus testovan\u00fd pomocou DraI PCR odhalil \u010fal\u0161\u00edch 7 pozit\u00edvnych vzoriek. Sh110\/SmSl PCR uk\u00e1zala, \u017ee 6 zo 7 slim\u00e1kov pozit\u00edvnych na DraI bolo infikovan\u00fdch parazitom S. haematobium. Pozit\u00edvne vzorky poch\u00e1dzali z riek Mbararani, Buburu, Kiwumiro a Bangoni.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Diskusia<\/strong><\/p>\n<p>Urogenit\u00e1lna schistosom\u00f3za je z\u00e1va\u017en\u00e9 chronick\u00e9 parazit\u00e1rne ochorenie s endemick\u00fdm v\u00fdskytom v mnoh\u00fdch rur\u00e1lnych oblastiach Kene. Snaha o kontrolu tohto ochorenia spo\u010d\u00edva<br \/>\ndodnes v pod\u00e1van\u00ed jednej d\u00e1vky praziquantelu raz ro\u010dne \u0161kolopovinn\u00fdm de\u0165om. Zabr\u00e1ni\u0165 op\u00e4tovn\u00e9mu styku det\u00ed po podanej lie\u010dbe s potenci\u00e1lne infikovanou vodou je v t\u00fdchto oblastiach z mnoh\u00fdch d\u00f4vodov neuskuto\u010dnite\u013en\u00e9. Cie\u013eom pri mana\u017emente schistosom\u00f3zy by malo by\u0165 nielen pod\u00e1vanie lie\u010diva proti parazit\u00f3ze na pravidelnej b\u00e1ze, ale aj preru\u0161enie prenosu v\u00fdvojov\u00fdch \u0161t\u00e1di\u00ed parazita (cerk\u00e1ri\u00ed) z medzihostite\u013ea na \u010dloveka. Mapovanie zamorenosti pou\u017e\u00edvan\u00fdch vodn\u00fdch zdrojov infikovan\u00fdm medzihostite\u013eom by mohlo ma\u0165 preto v mana\u017emente tohto ochorenia z\u00e1sadn\u00fd v\u00fdznam.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Z\u00e1ver<\/strong><\/p>\n<p>Mapovanie stavu zamorenosti vybran\u00fdch vodn\u00fdch zdrojov v regi\u00f3ne Kwale, Ke\u0148a dok\u00e1zalo pr\u00edtomnos\u0165 ulitn\u00edkov B. globosus, infikovan\u00fdch p\u00f4vodcom urogenit\u00e1lnej schistosom\u00f3zy. Zd\u00e1 sa, \u017ee \u010dasto pou\u017e\u00edvan\u00fd test uvo\u013e\u0148ovania cerk\u00e1ri\u00ed neodr\u00e1\u017ea skuto\u010dn\u00fa mieru infekcie, pri\u010dom prepatentn\u00e1 peri\u00f3da infekcie zachyten\u00e1 pomocou PCR testov by mohla by\u0165 v lep\u0161om vz\u0165ahu so skuto\u010dnou hladinou kontamin\u00e1cie vody parazitom.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>LITERAT\u00daRA<\/strong>1. Efforts to control schistosomosis in this area have been relying exclusively on the mass administration of praziquantel to school-age children once a year. Therefore, we tried to confirm\/exclude the possibility of reinfection of the population by monitoring the infestation of selected water resources with Bulinus globosus (intermediate host of disease), infected by parasite Schistosoma haematobium. B. globosus was identified in 10 out of 16 water sources, which were monitored. The cercariae shedding test was positive at two sites on the Pengo and Tsanganyiko rivers.
