Prv\u00fd \u00fadaj poch\u00e1dza z roku 1987, ke\u010f bolo diagnostikovan\u00fdch sedem pr\u00edpadov ochorenia sp\u00f4soben\u00fdch Cryptosporidium spp. u pacientov s HIV\/AIDS(1). Druh\u00fd pr\u00edpad bol zaznamenan\u00fd u dvoch s\u00farodencov v roku 2013, z ktor\u00fdch len sedemro\u010dn\u00fd chlapec vykazoval klinick\u00e9 pr\u00edznaky ochorenia. Pomocou DNA typiz\u00e1cie bol u t\u00fdchto det\u00ed identifikovan\u00fd druh C. hominis(2). V s\u00fa\u010dasnosti s\u00fa zn\u00e1me aj \u010fal\u0161ie infekcie det\u00ed z r\u00f3mskych os\u00e1d \u010di detsk\u00fdch domovov na v\u00fdchodnom Slovensku, ktor\u00e9 boli vyvolan\u00e9 najm\u00e4 druhmi C. hominis a C. muris(3). V na\u0161om pr\u00edspevku uv\u00e1dzame prv\u00fd pr\u00edpad \u013eudskej kryptosporidi\u00f3zy vyvolanej druhom C. parvum u imunokompetentn\u00e9ho pacienta na Slovensku.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Vlastn\u00fd pr\u00edpad<\/strong><\/p>\n<p>Pacient, 23-ro\u010dn\u00fd mu\u017e, v septembri 2018 prejavoval typick\u00e9 klinick\u00e9 pr\u00edznaky kryptosporidi\u00f3zy. Vo Fakultnej nemocnici v Ko\u0161iciach bol 4 dni hospitalizovan\u00fd s boles\u0165ou brucha, hna\u010dkou a \u00fabytkom hmotnosti. Po\u010das hospitaliz\u00e1cie dost\u00e1val pacient podporn\u00fa lie\u010dbu (di\u00e9ta, inf\u00fazie). Po kompletnom vy\u0161etren\u00ed nebol zisten\u00fd p\u00f4vodca bakteri\u00e1lneho ani v\u00edrusov\u00e9ho ochorenia. Gastrointestin\u00e1lne pr\u00edznaky trvali 7 dn\u00ed, potom sa stav pacienta postupne zlep\u0161oval. Po hospitaliz\u00e1cii pacient priniesol vzorky stolice na \u00dastav parazitol\u00f3gie UVLF v Ko\u0161iciach. Vzorku stolice sme vy\u0161etrili flota\u010dnou met\u00f3dou pod\u013ea Fausta s negat\u00edvnym v\u00fdsledkom. N\u00e1sledne sme zhotovili tenk\u00fd n\u00e1ter stolice, ktor\u00fd sme ofarbili \u0161peci\u00e1lnym acidorezistentn\u00fdm farben\u00edm pod\u013ea Ziehleho-Neelsena. Mikroskopick\u00fdm vy\u0161etren\u00edm boli identifikovan\u00e9 oocysty kryptospor\u00eddi\u00ed, ktor\u00e9 sa javili ako ru\u017eovo-fialov\u00e9 subsf\u00e9rick\u00e9 \u00fatvary na modrozelenom pozad\u00ed. Na potvrdenie suspektnej kryptosporidi\u00f3zy sme urobili ELISA test na d\u00f4kaz koproantig\u00e9nu s pozit\u00edvnym v\u00fdsledkom. Na extrakciu DNA z pozit\u00edvnej vzorky sme pou\u017eili komer\u010dn\u00fa s\u00fapravu ZR Fecal DNA MiniPrep\u2122 kit a n\u00e1sledne sme izol\u00e1ty podrobili genotypiz\u00e1cii pomocou \u00fasekov g\u00e9nov SSU rRNA a GP60(4). Na z\u00e1klade nested PCR sme amplifikovali mal\u00fa ribozom\u00e1lnu podjednotku SSU rRNA, \u010d\u00edm sme potvrdili druh C. parvum a n\u00e1sledne pomocou sekven\u00e1cie g\u00e9nu GP60 sme ur\u010dili zoonotick\u00fd subtyp IIdA15G1.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Diskusia<\/strong><\/p>\n<p>V na\u0161om pr\u00edspevku uv\u00e1dzame prv\u00fd pr\u00edpad \u013eudskej kryptosporidi\u00f3zy vyvolanej druhom C. parvum subtyp IIdA15G1 u imunokompetentn\u00e9ho pacienta na Slovensku. Dvadsa\u0165tri ro\u010dn\u00fd pacient v anamn\u00e9ze uviedol pr\u00e1cu s te\u013eatami na farme v obci Zempl\u00ednska Teplica (okres Trebi\u0161ov). Pravdepodobn\u00fd prenos infek\u010dn\u00fdch ooc\u00fdst nastal priamym kontaktom s infikovan\u00fdmi te\u013eatami a n\u00e1slednou nedostato\u010dnou hygienou r\u00fak. Vzh\u013eadom na tieto skuto\u010dnosti sme sa rozhodli vy\u0161etri\u0165 vzorky trusu teliat na farme v obci Zempl\u00ednska Teplica, kde sa pacient pracovne zdr\u017eiaval. Koproantig\u00e9ny boli potvrden\u00e9 ELISA testom v 5\/18 vzork\u00e1ch trusu. PCR met\u00f3dou sme zistili, \u017ee pozit\u00edvne izol\u00e1ty patrili k zoonotick\u00fdch subtypom rodiny IIa a IId. Vo 4 vzork\u00e1ch bol identifikovan\u00fd C. parvum IIdA17G1. V jednej vzorke bol potvrden\u00fd subtyp IIaA17G1R1(5). Z toho vypl\u00fdva, \u017ee te\u013eat\u00e1 predstavuj\u00fa v\u00fdznamn\u00fd rezervo\u00e1r zoonotick\u00fdch subtypov C. parvum aj pre imunokompetentn\u00fdch \u013eud\u00ed. Riziko hroz\u00ed predov\u0161etk\u00fdm po\u013enohospod\u00e1rom, veterin\u00e1rnym \u0161tudentom a lek\u00e1rom, zootechnikom a ostatn\u00fdm zamestnancom pracuj\u00facim na farme, ktor\u00ed prich\u00e1dzaj\u00fa do kontaktu s hospod\u00e1rskymi zvieratami.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Z\u00e1ver<\/strong><\/p>\n<p>Hov\u00e4dz\u00ed dobytok predstavuje jeden z naj\u010dastej\u0161\u00edch zdrojov kryptosporidi\u00f3zy u \u013eud\u00ed(5). Subtyp IIdA15G1, potvrden\u00fd na na\u0161om \u00fazem\u00ed, sa vyskytuje vo svete sporadicky a vyvol\u00e1va u pacientov miernu a\u017e stredne \u0165a\u017ek\u00fa hna\u010dku. Zdokumentovan\u00e9 pr\u00edpady boli hl\u00e1sen\u00e9 u \u013eud\u00ed a teliat v \u00c1zii a Eur\u00f3pe. Nato, aby sme pochopili molekul\u00e1rnu epidemiol\u00f3giu druhu C. parvum, je nevyhnutn\u00e9 identifikova\u0165 izol\u00e1ty na \u00farovni subtypov.
