Prostatick\u00fd karcin\u00f3m sa v\u0161ak m\u00f4\u017ee spr\u00e1va\u0165 aj agres\u00edvne. Spo\u013eahliv\u00e9 predikt\u00edvne a prognostick\u00e9 biomarkery s\u00fa nevyhnutn\u00e9 na odl\u00ed\u0161enie menej a viac agres\u00edvnych karcin\u00f3mov a na selekciu pacientov, ktor\u00ed vy\u017eaduj\u00fa akt\u00edvnu lie\u010dbu, od t\u00fdch, u ktor\u00fdch je v danom \u010dase dosta\u010duj\u00faca <em>active surveillance. <\/em>Neadekv\u00e1tna lie\u010dba m\u00f4\u017ee vies\u0165 k zhor\u0161eniu kvality \u017eivota pacientov alebo by\u0165 priamo fat\u00e1lna. V s\u00fa\u010dasnosti dostupn\u00e9 prognostick\u00e9 biomarkery ako s\u00e9rov\u00e1 hladina PSA, \u0161t\u00e1dium choroby a Gleasonovo sk\u00f3re sl\u00fa\u017eia s\u00edce na stratifik\u00e1ciu pacientov do rizikov\u00fdch skup\u00edn, nie s\u00fa v\u0161ak dostato\u010dne presn\u00e9. H\u013eadanie biomarkerov karcin\u00f3mu prostaty, ktor\u00e9 by vedeli spo\u013eahlivo \u201epredpoveda\u0165\u201c agresivitu n\u00e1doru, je teda st\u00e1le aktu\u00e1lne.<\/p>\n<p>Na identifik\u00e1ciu potenci\u00e1lnych biomarkerov v poslednom \u010dase v\u00fdraznou mierou prispievaj\u00fa nov\u00e9 met\u00f3dy molekulovej genetiky. Kristiansen a spol.(1) identifikovali pri porovnan\u00ed profilov ben\u00edgneho tkaniva prostaty s karcin\u00f3mom odli\u0161n\u00fdch 98 g\u00e9nov, ktor\u00e9 by mohli zohr\u00e1va\u0165 \u00falohu v adenokarcin\u00f3me prostaty. G\u00e9ny z tohto zoznamu podnecuj\u00fa v s\u00fa\u010dasnosti v\u00fdskum biomarkerov rakoviny prostaty.<\/p>\n<p>Medzi identifikovan\u00e9 g\u00e9ny patr\u00ed aj g\u00e9n pre tetraspan\u00edn 1 (Tspan1), \u010dlen rodiny tetraspan\u00ednov. Superrodinu tetraspan\u00ednov tvor\u00ed 33 transmembr\u00e1nov\u00fdch glykoprote\u00ednov, ktor\u00e9 s\u00fa schopn\u00e9 vytv\u00e1ra\u0165 komplexy, membr\u00e1nov\u00e9 mikrodom\u00e9ny obohaten\u00e9 o tetraspan\u00edny (TEMs) s in\u00fdmi tetraspan\u00ednmi alebo s in\u00fdmi transmembr\u00e1nov\u00fdmi a cytoplazmov\u00fdmi prote\u00ednmi. T\u00e1to sie\u0165 tetraspan\u00ednov m\u00f4\u017ee ovplyvni\u0165 prenos sign\u00e1lu do bunky a zohr\u00e1va \u00falohu v \u0161irokej \u0161k\u00e1le biologick\u00fdch procesov zah\u0155\u0148uj\u00facich prolifer\u00e1ciu buniek, bunkov\u00fa adh\u00e9ziu, migr\u00e1ciu a motilitu buniek(2,3).<\/p>\n<p>Zv\u00fd\u0161en\u00e1 expresia tetraspan\u00ednov bola preuk\u00e1zan\u00e1 v r\u00f4znych mal\u00edgnych n\u00e1doroch zahr\u0148uj\u00facich karcin\u00f3my kr\u010dka maternice, endometria, prsn\u00edka, p\u013e\u00fac, hrub\u00e9ho \u010dreva, pankreasu a in\u00e9(3-8). \u00daloha tetraspan\u00ednov v karcinogen\u00e9ze sa zd\u00e1 mnohotv\u00e1rna. \u0160t\u00fadie preuk\u00e1zali, \u017ee v mal\u00edgnych n\u00e1doroch s\u00fa tetraspan\u00edny zapojen\u00e9 do procesu prolifer\u00e1cie, migr\u00e1cie a inv\u00e1zie n\u00e1dorov\u00fdch buniek a m\u00f4\u017eu ma\u0165 pri tom podporn\u00fa alebo supres\u00edvnu \u00falohu(3).<\/p>\n<p>\u0160t\u00fadie nazna\u010duj\u00fa aj mo\u017en\u00fa \u00falohu tetraspan\u00ednov v adenokarcin\u00f3me prostaty. Munkley et al.(9) preuk\u00e1zali, upregul\u00e1ciu g\u00e9nu Tspan1 v n\u00e1dorovom tkanive prostaty v porovnan\u00ed s nen\u00e1dorov\u00fdm a jeho spojitos\u0165 s migr\u00e1ciou n\u00e1dorov\u00fdch buniek, a to pod kontrolou androg\u00e9nov.<\/p>\n<p>Cie\u013eom tejto pr\u00e1ce bolo hodnotenie imunohistochemickej expresie prote\u00ednu Tspan1 v bioptick\u00fdch vzork\u00e1ch adenokarcin\u00f3mov prostaty, porovnanie s nen\u00e1dorov\u00fdm tkanivom prostaty aj porovnanie expresie vzh\u013eadom na prognostick\u00e9 ukazovatele ako Gleasonovo sk\u00f3re a \u0161t\u00e1dium choroby.<\/p>\n<p>&nbsp;<\/p>\n<h2>Materi\u00e1l a metodika<\/h2>\n<p><strong>\u00a0<\/strong><\/p>\n<h3>S\u00fabor pacientov<\/h3>\n<p>S\u00fabor pacientov tvorilo spolu 21 mu\u017eov vo veku od 55 do 74 rokov, u ktor\u00fdch bola vykonan\u00e1 radik\u00e1lna prostatekt\u00f3mia na Urologickej klinike Fakultnej nemocnice Trnava pre biopticky potvrden\u00fd adenokarcin\u00f3m prostaty. Kontroln\u00fa skupinu tvorilo 5 pacientov po TUR-P s klinick\u00fdm a bioptick\u00fdm n\u00e1lezom ben\u00edgnej hyperpl\u00e1zie prostaty. Klinickopatologick\u00e9 charakteristiky adenokarcin\u00f3mov prostaty s\u00fa uveden\u00e9 <strong>v tabu\u013eke 1<\/strong>.<\/p>\n<p>&nbsp;<\/p>\n<h3>Tkanivov\u00e9 microarray (TMA) blo\u010dky<\/h3>\n<p>Na imunohistochemick\u00e9 vy\u0161etrenie boli pou\u017eit\u00e9 tkanivov\u00e9 microarray pripraven\u00e9 z FFPE vzoriek prost\u00e1t. Od ka\u017ed\u00e9ho pacienta boli analyzovan\u00e9 dve vzorky karcin\u00f3mu z r\u00f4znych miest tkaniva prostaty, aby sa podchytila heterogenita n\u00e1doru. TMA vzorky s priemerom 3 mm boli odobran\u00e9 punk\u010dn\u00fdm skalpelom Harris Uni-Core (Sigma-Aldrich, Steinheim, Germany). Na imunohistochemick\u00e9 farbenie sa z microarray blo\u010dkov narezali 4-5 \u03bcm rezy tkaniva.<\/p>\n<p>&nbsp;<\/p>\n<h3>Imunohistochemick\u00e9 farbenie<\/h3>\n<p>Po deparafiniz\u00e1cii a revitaliz\u00e1cii s pou\u017eit\u00edm Dako PT Link (Agilent Dako, Santa Clara, CA, USA) s roztokom EnVision\u2122 FLEX Target Retrieval Solution, pH 9 (Agilent Dako, Santa Clara, CA, USA) boli prepar\u00e1ty v imunostaineri (Autostainer Link 48, Agilent Dako, Santa Clara, CA, USA) inkubovan\u00e9 s prim\u00e1rnou protil\u00e1tkou (Tspan1, klon ab254730, Abcam, Cambridge, UK, polyklon\u00e1lna kr\u00e1li\u010dia, riedenie 1 : 500) po\u010das 60\u00a0min\u00fat, n\u00e1sledne so sekund\u00e1rnou protil\u00e1tkou a nakoniec farben\u00e9 diaminobenzid\u00ednom DAB+ (Agilent Dako, Santa Clara, CA, USA) a dofarben\u00e9 H &amp; E.<\/p>\n<p>Ako negat\u00edvnu kontrolu sme pou\u017eili rezy inkubovan\u00e9 s riediacim roztokom bez prim\u00e1rnej protil\u00e1tky. Ako pozit\u00edvnu kontrolu sme pou\u017eili vzorky tkaniva obli\u010dky a hrub\u00e9ho \u010dreva.<\/p>\n<p>&nbsp;<\/p>\n<h3>Hodnotenie imunohistochemick\u00e9ho farbenia<\/h3>\n<p>Ako pozit\u00edvnu sme hodnotili cytoplazmov\u00fa expresiu Tspan1, pri\u010dom sme stanovili histosk\u00f3re na z\u00e1klade percenta pozit\u00edvnych buniek (0 = 0 % pozit\u00edvnych buniek, 1 = &lt; 10 % pozit\u00edvnych buniek, 2 = 10 \u2013 50 % pozit\u00edvnych buniek, 3 = &gt; 50 % pozit\u00edvnych buniek) a intenzity farbenia (0 = negat\u00edvne, 1 = slab\u00e1 pozitivita, 2 = stredne siln\u00e1 pozitivita, 3 = siln\u00e1 pozitivita) pod\u013ea Xu et al. (10). Pod\u013ea v\u00fdsledn\u00e9ho histosk\u00f3re boli vzorky zaraden\u00e9 do kateg\u00f3rie s n\u00edzkou (+, histosk\u00f3re \u2264 2), so strednou (++, histosk\u00f3re 3-5) alebo s vysokou (+++, histosk\u00f3re \u2265 6) mierou expresie Tspan1.<\/p>\n<p>&nbsp;<\/p>\n<h3>V\u00fdsledky<\/h3>\n<p>V\u0161etky vy\u0161etren\u00e9 vzorky boli pozit\u00edvne na prote\u00edn Tspan1 s r\u00f4znou mierou expresie. Pr\u00edpady ben\u00edgnej hyperpl\u00e1zie prostaty vykazovali slab\u00fa expresiu Tspan1 <strong>(obr\u00e1zok 1)<\/strong>, pri\u010dom expresia bola preva\u017ene lumin\u00e1lna a len fok\u00e1lne cytoplazmov\u00e1.<\/p>\n<p>V adenokarcin\u00f3moch prostaty bola pozitivita Tspan1 preva\u017ene cytoplazmov\u00e1, niekedy s pridru\u017eenou lumin\u00e1lou expresiou a\/alebo intralumin\u00e1lnou pozitivitou sekr\u00e9tu. Adenokarcin\u00f3my prostaty vykazovali r\u00f4znu mieru expresie Tspan1, od n\u00edzkej (+) v 33,3 % pr\u00edpadov <strong>(obr\u00e1zok 2) <\/strong>cez stredn\u00fa (++) v 52,4 % pr\u00edpadov a\u017e po vysok\u00fa (+++) v 14,3 % pr\u00edpadov <strong>(obr\u00e1zok 3)<\/strong>.<\/p>\n<p>Vzh\u013eadom na diferencovanos\u0165 n\u00e1dorov vykazovali adenokarcin\u00f3my s ni\u017e\u0161\u00edm Gleasonov\u00fdm sk\u00f3re (GS 6-7) preva\u017ene stredn\u00fa mieru expresie (++), n\u00e1dory s Gleasonov\u00fdm sk\u00f3re 8 stredn\u00fa a\u017e vysok\u00fa mieru expresie (+++) a m\u00e1lo diferencovan\u00e9 n\u00e1dory s Gleasonov\u00fdm sk\u00f3re 9 preva\u017ene n\u00edzku mieru expresie (+) Tspan1. Podobn\u00fd trend expresie vykazovali adenokarcin\u00f3my vzh\u013eadom na WHO\/ISUP grade.<\/p>\n<p>Vzh\u013eadom na patologick\u00e9 \u0161t\u00e1dium choroby (pT) vykazovali lok\u00e1lne pokro\u010dil\u00e9 adenokarcin\u00f3my prostaty (pT3b) \u010dastej\u0161ie n\u00edzku mieru expresie Tspan1 v porovnan\u00ed s n\u00e1dormi lokalizovan\u00fdmi na tkanivo prostaty (pT2).<\/p>\n<p>Sum\u00e1r imunohistochemickej expresie Tspan1 v adenokarcin\u00f3moch prostaty je uveden\u00fd <strong>v tabu\u013eke 1<\/strong>.<\/p>\n<p>&nbsp;<\/p>\n<h3>Diskusia<\/h3>\n<p>Adenokarcin\u00f3m prostaty je \u010dast\u00e9 n\u00e1dorov\u00e9 ochorenie. V\u00e4\u010d\u0161ina pr\u00edpadov je m\u00e1lo agres\u00edvnych, pomaly rast\u00facich. V \u010dasti pr\u00edpadov sa v\u0161ak adenokarcin\u00f3m prostaty spr\u00e1va agres\u00edvne. V s\u00fa\u010dasnosti dostupn\u00e9 biomarkery nie s\u00fa dostato\u010dne presn\u00e9 na selekciu pacientov, \u010do m\u00f4\u017ee vies\u0165 k podlie\u010deniu agres\u00edvnych karcin\u00f3mov alebo nadlie\u010deniu neagres\u00edvnych n\u00e1dorov, a tak m\u00e1 nemal\u00fd vplyv na kvalitu \u017eivota pacienta. Snaha n\u00e1js\u0165 \u010do najpresnej\u0161ie a najspo\u013eahlivej\u0161ie predikt\u00edvne a prognostick\u00e9 biomarkery adenokarcin\u00f3mu prostaty st\u00e1le podnecuje nov\u00fd v\u00fdskum. Met\u00f3dy molekulovej genetiky umo\u017e\u0148uj\u00fa identifikova\u0165 g\u00e9ny asociovan\u00e9 s tumorom. V adenokarcin\u00f3me prostaty medzi tieto g\u00e9ny patr\u00ed aj Tspan1, k\u00f3duj\u00faci transmebr\u00e1nov\u00fd prote\u00edn Tspan1, \u010dlen rodiny tetraspan\u00ednov.<\/p>\n<p>Tetraspan\u00edny zohr\u00e1vaj\u00fa \u00falohu v \u0161irokej \u0161k\u00e1le biologick\u00fdch procesov zahr\u0148uj\u00facich predov\u0161etk\u00fdm proces migr\u00e1cie a motility, a to v bunk\u00e1ch norm\u00e1lnych, ale aj mal\u00edgne transformovan\u00fdch. Migr\u00e1cia buniek je z\u00e1kladn\u00fdm procesom inv\u00e1zie a metastatickej progresie rakoviny. Zmeny expresie tetraspan\u00ednov boli op\u00edsan\u00e9 v r\u00f4znych karcin\u00f3moch vr\u00e1tane adenokarcin\u00f3mu prostaty(3,9).<\/p>\n<p>Imunohistochemick\u00fa expresiu Tspan1 sme\u00a0 hodnotili v bioptick\u00fdch vzork\u00e1ch acin\u00e1rnych adenokarcin\u00f3mov a ben\u00edgnej hyperpl\u00e1zie prostaty. V\u0161etky vy\u0161etren\u00e9 vzorky boli pozit\u00edvne na prote\u00edn Tspan1, vykazovali v\u0161ak r\u00f4znu mieru expresie. Tspan1 exprimuj\u00fa aj ben\u00edgne tkaniv\u00e1, ale zdanlivo v men\u0161ej miere, respekt\u00edve v ni\u017e\u0161ej intenzite(8,10), \u010do preukazuj\u00fa aj na\u0161e v\u00fdsledky. Pr\u00edpady ben\u00edgnej hyperpl\u00e1zie prostaty vykazovali slab\u00fa (+) expresiu Tspan1, ktor\u00e1 bola fok\u00e1lna a preva\u017ene lumin\u00e1lna, k\u00fdm v karcin\u00f3moch bola expresia preva\u017ene cytoplazmov\u00e1 s pridru\u017eenou lumin\u00e1lou expresiou a\/alebo intralumin\u00e1lnou pozitivitou sekr\u00e9tu. Podobn\u00fa kompartment\u00e1ciu Tspan1 pozitivity opisuje vo svojej \u0161t\u00fadii Stinnesbeck et al.(11).<\/p>\n<p>Adenokarcin\u00f3my prostaty vykazovali r\u00f4znu mieru expresie Tspan1, od n\u00edzkej (+) a\u017e po vysok\u00fa (+++), pri\u010dom vo vy\u0161e polovici pr\u00edpadov (52,4 %) sme zaznamenali expresiu strednej miery (++). Overexpresiu g\u00e9nu Tspan1 (na \u00farovni mRNA) v n\u00e1dorovom tkanive v porovnan\u00ed s ben\u00edgnym tkanivom prostaty potvrdil Stinnesbeck et al.(11). Pri imunohistochemickej anal\u00fdze na \u00farovni prote\u00ednovej expresie v\u0161ak zistili, \u017ee \u010das\u0165 adenokarcin\u00f3mov (35 %) vykazuje porovnate\u013ene n\u00edzku expresiu ako ben\u00edgne tkanivo prostaty. V na\u0161om s\u00fabore sme n\u00edzku mieru expresie Tspan1 zaznamenali v 33,3 % pr\u00edpadov (n = 7) adenokarcin\u00f3mov. T\u00e1to n\u00edzka miera expresie Tspan1 v \u010dasti adenokarcin\u00f3mov teda limituje jeho vyu\u017eitie ako diagnostick\u00e9ho markera, i ke\u010f sme pozorovali odli\u0161n\u00fa lokaliz\u00e1ciu pozitivity v ben\u00edgnom tkanive (preva\u017ene lumin\u00e1lna) v porovnan\u00ed s mal\u00edgnym (preva\u017ene cytoplazmov\u00e1).<\/p>\n<p>Na pos\u00fadenie prognostick\u00e9ho potenci\u00e1lu Tspan1 sme korelovali mieru expresie tohto prote\u00ednu so zn\u00e1mymi prognostick\u00fdmi ukazovate\u013emi, predov\u0161etk\u00fdm s Gleasonov\u00fdm sk\u00f3re a so \u0161t\u00e1diom choroby (pT). K\u00fdm adenokarcin\u00f3my s ni\u017e\u0161\u00edm Gleasonov\u00fdm sk\u00f3re (GS 6-7) vykazovali preva\u017ene stredn\u00fa mieru (++) expresie Tspan1, n\u00e1dory s Gleasonov\u00fdm sk\u00f3re 8 \u010dastej\u0161ie vysok\u00fa mieru (+++) expresie a m\u00e1lo diferencovan\u00e9 n\u00e1dory s Gleasonov\u00fdm sk\u00f3re 9 preva\u017ene n\u00edzku mieru (+) expresie Tspan1. Signifikantn\u00fa inverzn\u00fa korel\u00e1ciu medzi n\u00e1dorov\u00fdm grade a imunohistochemickou expresiou Tspan1 zaznamenal aj Stinnesbeck et al.(11).<\/p>\n<p>Vzh\u013eadom na patologick\u00e9 \u0161t\u00e1dium choroby (pT) vykazovali lok\u00e1lne pokro\u010dil\u00e9 adenokarcin\u00f3my prostaty (pT3b) \u010dastej\u0161ie n\u00edzku mieru expresie Tspan1 v porovnan\u00ed s n\u00e1dormi lokalizovan\u00fdmi na tkanivo prostaty (pT2). Zn\u00ed\u017eenie expresie Tspan1 v pokro\u010dil\u00fdch n\u00e1doroch zaznamenal aj Xu et al.(10) spolu so signifikantnou korel\u00e1ciou tejto zn\u00ed\u017eenej expresie so v\u010dasnou biochemickou recid\u00edvou choroby. Spojitos\u0165 zn\u00ed\u017eenej g\u00e9novej expresie Tspan1 s pokro\u010dil\u00fdm \u0161t\u00e1diom choroby preuk\u00e1zali aj Munkley et al.(9), ktor\u00ed pri porovn\u00e1van\u00ed expresie Tspan1 v prim\u00e1rnom adenokarcin\u00f3me prostaty s metast\u00e1zou pozorovali v\u00fdrazn\u00fd pokles expresie v n\u00e1dorov\u00fdch bunk\u00e1ch metast\u00e1zy.<\/p>\n<p>Zisten\u00e1 spojitos\u0165 zn\u00ed\u017eenej expresie Tspan1 s vy\u0161\u0161\u00edm Gleasonov\u00fdm sk\u00f3re a lok\u00e1lne pokro\u010dilou chorobou by nazna\u010dovala negat\u00edvny prognostick\u00fd potenci\u00e1l Tspan1 v adenokarcin\u00f3me prostaty, prispieva tak k poznatkom o biomarkeroch karcin\u00f3mu prostaty a predstavuje podnet na nadv\u00e4zuj\u00face v\u00fdskumy.<\/p>\n<p>&nbsp;<\/p>\n<p><strong><em>Po\u010fakovanie<\/em><\/strong><\/p>\n<p><em>\u010cl\u00e1nok vznikol s podporou projektu VEGA 1\/0684\/21 <\/em><em>\u201ePrognostick\u00fd v\u00fdznam zmien inerv\u00e1cie pri hyperpl\u00e1zii a neopl\u00e1zii prostaty\u201c.<\/em><\/p>\n<p>&nbsp;<\/p>\n<p><strong>LITERAT\u00daRA<\/strong><\/p>\n<ol>\n<li>Kristiansen G, Pilarsky C, Wissmann C, et Expression pro\ufb01ling of microdissected matched prostate cancer samples reveals CD166\/MEMD and CD24 as new prognostic markers for patient survival. Therefore identification of reliable biomarkers of prostate cancer aggressiveness is still an actual topic. Tetraspanin 1 (TSPAN1) was found to be a tumour associated gene in prostate cancer. This study focused on evaluating immunohistochemical Tspan1 expression in samples of prostate adenocarcinoma (CaP) and benign prostatic hyperplasia (BPH). All samples were positive for Tspan1; showed, however, different degree of expression, BHP being only weakly, focally positive with mainly luminal expression and CaP showing cytoplasmic positivity of low to a high degree. The lower degree of Tspan 1 expression was found in CaP cases with higher Gleason score and locally advanced disease.
