Jeho v\u00fdskyt v popul\u00e1cii st\u00e1le rastie. Za posledn\u00fdch 30 rokov pre- valencia diabetu mellitu (bez oh\u013eadu na typ) st\u00fapla dvojn\u00e1sobne. Jeho z\u00e1va\u017enos\u0165 zd\u00f4raz\u0148uje aj vy\u0161\u0161ia prevalenia v Slovenskej republike v porovnan\u00ed s priemerom v Eur\u00f3pskej \u00fanii a rast po\u010dtu nov\u00fdch pr\u00edpadov(1). Probl\u00e9mov\u00e9 s\u00fa pr\u00e1ve komplik\u00e1cie diabetu mellitu s\u00favisiace s cievnymi zmenami, neuropatiou a celkov\u00fdmi zmenami metabolizmu. Vhodn\u00e1 diagn\u00f3za a lie\u010dba m\u00f4\u017eu zabr\u00e1ni\u0165 rozsiahlemu po\u0161kodeniu dolnej kon\u010datiny s vysok\u00fdmi n\u00e1kladmi na lie\u010dbu (<strong>obr\u00e1zok 1 a 2<\/strong>).<\/p>\n<p>Diabetick\u00e1 noha, ako aj in\u00e9 \u0165a\u017eko sa hojace vredy asociovan\u00e9 s diabetom mellitom, ktor\u00e9 nezriedka progreduj\u00fa do gangr\u00e9ny, patria k \u010dast\u00fdm komplik\u00e1ci\u00e1m zle kompenzovan\u00e9ho diabetu mellitu a v\u00e1\u017enym sp\u00f4sobom zdravotne za\u0165a\u017euj\u00fa pacienta a jeho rodinu. M\u00f4\u017eu vies\u0165 k strat\u00e1m postihnut\u00fdch kon\u010dat\u00edn a invalidizova\u0165 pacienta.<\/p>\n<p>Vznik diabetickej nohy s\u00favis\u00ed s kombin\u00e1ciou zmien vyvolan\u00fdch diabetom mellitom, prim\u00e1rne s neuropatiou a s \u0148ou s\u00favisiacim vy\u0161\u0161\u00edm rizikom vzniku drobn\u00fdch r\u00e1n, so zmenen\u00fdm mikrobi\u00f3mom ko\u017ee(2) so sekund\u00e1rnou infekciou r\u00e1n, s naru\u0161enou imunitnou reakciou organizmu a celkov\u00fdm ischemick\u00fdm prostred\u00edm v d\u00f4sledku arteri\u00e1lnej okluz\u00edvnej choroby. Pr\u00e1ve opakovan\u00e9 traumy pri ch\u00f4dzi spolu so zn\u00ed\u017eenou citlivos\u0165ou pri neuropatii predisponuj\u00fa na poranenie ko\u017ee a sp\u00f4sobuj\u00fa \u00fabytok a dislok\u00e1ciu ochrann\u00fdch tukov\u00fdch vank\u00fa\u0161ikov. To pri nedostato\u010dnej ochrane ko\u017ee alebo nevhodnej obuvi vedie k vzniku vredov a k infekci\u00e1m(3,4).<\/p>\n<p>Ve\u013emi dobre je op\u00edsan\u00fd podiel neuropatie a vaskulopatie na vzniku diabetickej nohy. Diabetick\u00e1 neuropatia vedie k postihnutiu motorick\u00fdch, senzorick\u00fdch aj auton\u00f3mnych nervov\u00fdch vl\u00e1kien. \u010casto b\u00fdvaj\u00fa prim\u00e1rne postihnut\u00e9 najdlh\u0161ie perif\u00e9rne nervy. Strata reflexu Achillovej \u0161\u013eachy je najskor\u0161\u00edm pr\u00edznakom t\u00fdchto zmien. Denerv\u00e1cia, resp. po\u0161koden\u00e1 inerv\u00e1cia sp\u00f4sobuje atrofiu interose\u00e1lnych svalov nohy, \u010do sa podie\u013ea na anatomick\u00fdch zmen\u00e1ch klenby nohy, vzniku osteoartropatie (Charcotova noha) a deform\u00e1ci\u00e1ch prstov. Neuropatick\u00e9 zmeny na chodidle s\u00fa be\u017en\u00e9, diagn\u00f3za je \u010dasto oneskoren\u00e1. Kritick\u00e9 je aj naru\u0161enie senzorick\u00fdch nervov\u00fdch vl\u00e1kien so stratou propriocepcie, zmenou schopnosti vn\u00edmania tlaku, vibr\u00e1ci\u00ed aj bolestiv\u00fdch podnetov(3,5). Metabolick\u00e9 zmeny pri diabete, sprev\u00e1dzan\u00e9 hyperglyk\u00e9miou a oxida\u010dn\u00fdm vo\u013enoradik\u00e1lov\u00fdm pre\u0165a\u017een\u00edm organizmu ved\u00fa k endotelovej dysfunkcii. T\u00e1 v kombin\u00e1cii s hyperlipid\u00e9miou a so zmenami aktiv\u00e1cie trombocytov(6) akceleruje aterosklerotick\u00e9 zmeny u pacientov. Distrib\u00facia aterosklerotick\u00e9ho ochorenia doln\u00fdch kon\u010dat\u00edn u diabetikov sa l\u00ed\u0161i od tej nediabetikov. Prednostne postihuje dist\u00e1lne art\u00e9rie doln\u00fdch kon\u010dat\u00edn (tibi\u00e1lne art\u00e9rie) s menej \u010dast\u00fdm postihnut\u00edm femoropoplite\u00e1lneho syst\u00e9mu. Ischemiz\u00e1cia nohy sa podie\u013ea na vzniku ischemick\u00e9ho vredu a v pr\u00edpade z\u00e1va\u017en\u00fdch komplik\u00e1ci\u00ed aj gangr\u00e9ne(3,7,8).<\/p>\n<p>Napriek adekv\u00e1tnej lie\u010dbe je hojenie diabetick\u00e9ho vredu pomerne komplikovan\u00e9 a nie v\u017edy \u00faspe\u0161n\u00e9. Nie v\u017edy to mo\u017eno vysvetli\u0165 pretrv\u00e1vaj\u00facou ischemiz\u00e1ciou, ve\u013ek\u00e1 skupina neuropatick\u00fdch diabetick\u00fdch vredov nezriedka progreduje aj v pr\u00edpadoch, ke\u010f s\u00fa zachovan\u00e9 dostato\u010dn\u00e9 prietoky cievami a napriek adekv\u00e1tnej terapii.<\/p>\n<p>Hojenie je za fyziologick\u00fdch okolnost\u00ed pomerne presne definovan\u00fd konzervat\u00edvny proces. Po\u0161kodenie vedie k nekr\u00f3ze tkaniva a bakteri\u00e1lnej koloniz\u00e1cii sprev\u00e1dzanej ak\u00fatnym z\u00e1palom. Pr\u00e1ve z\u00e1palov\u00e1 reakcia je d\u00f4le\u017eit\u00e1 nielen na potla\u010denie bakteri\u00e1lneho rastu, vedie v\u0161ak aj k aktiv\u00e1cii tvorby cytok\u00ednov, ktor\u00e9 sa podie\u013eaj\u00fa na neoangiogen\u00e9ze, tvorbe nov\u00fdch ciev a ich rekanaliz\u00e1cii, migr\u00e1cii myofibroblastov a tvorbe v\u00e4zivovej jazvy.<\/p>\n<p>Cel\u00fd tento dej je pod kontrolou lok\u00e1lnych cytok\u00ednov a rastov\u00fdch faktorov, ktor\u00e9 ovplyv\u0148uj\u00fa aktivitu fibroblastov, schopnos\u0165 regener\u00e1cie epitelu aj endotelov\u00fdch buniek (IGF1), ovplyv\u0148uj\u00fa chemotaxiu z\u00e1palov\u00fdch buniek, ale aj myofibrob<\/p>\n<p>lastov, ich aktiv\u00e1ciu a diferenci\u00e1ciu i zlo\u017eenie extracelul\u00e1rneho matrixu (TGF-b1, PDGF, zlo\u017eky aktivovan\u00e9ho komplementu). Kontrolovan\u00e1 z\u00e1palov\u00e1 reakcia je k\u013e\u00fa\u010dov\u00e1 pre spr\u00e1vnu repar\u00e1ciu po\u0161koden\u00e9ho tkaniva, tvorbu granula\u010dn\u00e9ho tkaniva aj pre regener\u00e1ciu povrchov\u00e9ho epitelu a reepiteliz\u00e1ciu rany (<strong>obr\u00e1zok 3 a\u017e 5<\/strong>)(9,10).<\/p>\n<p>Diabetes mellitus ovplyv\u0148uje expresiu a aktivitu rastov\u00fdch faktorov. Je zn\u00e1me, \u017ee u pacientov s diabetom doch\u00e1dza k zn\u00ed\u017eenej aktiv\u00e1cii TGF-b1 a, naopak, zv\u00fd\u0161enej aktiv\u00e1cii TGF-b3. V d\u00f4sledku toho rastie aktivita ne\u0161pecifickej z\u00e1palovej reakcie a aktivita makrof\u00e1gov. Diabetes mellitus je charakteristick\u00fd n\u00e1padn\u00fdm z\u00e1palov\u00fdm fenotypom odpovede (<strong>obr\u00e1zok 6<\/strong>)(11).<\/p>\n<p>D\u00f4le\u017eit\u00fa \u00falohu v t\u00fdchto dejoch zohr\u00e1va aj tvorba oxidu dusnat\u00e9ho. K\u00fdm za fyziologick\u00fdch okolnost\u00ed je NO tvoren\u00fd preva\u017ene indukt\u00edvnou NO synt\u00e1zou NOS2 v kontrolovanom mno\u017estve a m\u00e1 priamy \u00fa\u010dinok na angiogen\u00e9zu a antimikrobi\u00e1lnu aktivitu, u pacientov s diabetom mellitom doch\u00e1dza v d\u00f4sledku nekontrolovanej z\u00e1palovej reakcie k premene NO, jeho zn\u00ed\u017eenej biodostupnosti a k zv\u00fd\u0161eniu tvorby reakt\u00edvnych vo\u013en\u00fdch radik\u00e1lov (<strong>obr\u00e1zok<\/strong><strong> 7<\/strong>). Zv\u00fd\u0161en\u00e1 tvorba ROS inhibuje angiogen\u00e9zu a spoma\u013euje procesy prepar\u00e1cie a regener\u00e1cie(12-15).<\/p>\n<p>V d\u00f4sledku zn\u00ed\u017eenej aktiv\u00e1cie TGF-\u03b21 a zv\u00fd\u0161enej aktiv\u00e1cie TGF-\u03b23 doch\u00e1dza aj k zmen\u00e1m remodel\u00e1cie extracelul\u00e1rnej hmoty u pacientov s diabetom mellitom. Zn\u00ed\u017een\u00e1 produkcia kolag\u00e9nu a, naopak, jeho zv\u00fd\u0161en\u00e9 odb\u00faravanie ako n\u00e1sledok poru\u0161enej rovnov\u00e1hy matrixov\u00fdch metaloprotein\u00e1z spoma\u013euje tvorbu jazvy a sp\u00f4sobuje zmenu jej funk\u010dn\u00fdch charakterist\u00edk(16-18) (<strong>obr\u00e1zok 8<\/strong>).<\/p>\n<p>&nbsp;<\/p>\n<h3>Pr\u00edpadov\u00e1 \u0161t\u00fadia<\/h3>\n<p>Prezentujeme pr\u00edpadov\u00fa \u0161t\u00fadiu 5 pacientov s diagn\u00f3zou diabetes mellitus 2. typu a s diabetickou ulcer\u00e1ciou. U pacientov boli bioptick\u00e9 vzorky ulcer\u00e1cie odobrat\u00e9 pred za\u010diatkom lie\u010dby. Pacienti boli n\u00e1sledne lie\u010den\u00ed \u0161tandardn\u00fdmi lie\u010debn\u00fdmi postupmi pou\u017e\u00edvan\u00fdmi v terapii chronick\u00fdch vredov, a to od\u013eah\u010den\u00edm kon\u010dat\u00edn, chirurgick\u00fdm vy\u010disten\u00edm rany a lok\u00e1lnou a celkovou antibiotickou lie\u010dbou. Ako kontroln\u00e9 boli analyzovan\u00e9 vzorky ulcer\u00e1ci\u00ed doln\u00fdch kon\u010dat\u00edn u 2 pacientov s neuropatick\u00fdmi vredmi, bez diagn\u00f3zy diabetes mellitus. Vo v\u0161etk\u00fdch vybrat\u00fdch pr\u00edpadoch sme pozorovali vaskularizovan\u00e9 granula\u010dn\u00e9 tkanivo so zmie\u0161anou z\u00e1palovou infiltr\u00e1ciou. Vybrat\u00e9 boli pr\u00edpady v podobnom \u0161t\u00e1diu hojenia, charakteristick\u00e9 zodpovedaj\u00facim podielom kolag\u00e9nu vo vyzrievaj\u00facom granula\u010dnom tkanive. V histologick\u00fdch vzor- k\u00e1ch u pacientov s diabetickou nohou bola pr\u00edtomn\u00e1 naru\u0161en\u00e1 angiogen\u00e9za, charakteristick\u00e1 men\u0161ou denzitou ciev, a zv\u00fd\u0161en\u00e1 expresia NOS2.<\/p>\n<p>&nbsp;<\/p>\n<h3>Z\u00e1ver<\/h3>\n<p>Diabetes mellitus je komplexn\u00e1 metabolick\u00e1 choroba maj\u00faca vplyv na regul\u00e1cie procesov regener\u00e1cie a repar\u00e1cie v tkanive. Diabetick\u00e1 noha predstavuje chorobu, ktor\u00e1 vznik\u00e1 v d\u00f4sledku poruchy regul\u00e1ci\u00ed, a to nielen v d\u00f4sledku diabetickej neuropatie a vaskulopatie, ale aj v s\u00favislosti s naru\u0161enou mikrobi\u00e1lnou ko\u017enou fl\u00f3rou a poru\u0161en\u00fdm procesom hojenia. Vedie k poruch\u00e1m regul\u00e1cie tvorby rastov\u00fdch faktorov spoma\u013euj\u00facich rast granula\u010dn\u00e9ho tkaniva, jeho vyzrievania a reepiteliz\u00e1cie povrchov\u00e9ho ko\u017en\u00e9ho krytu. Sp\u00f4sobuje vznik nekontrolovan\u00e9ho z\u00e1palov\u00e9ho fenotypu a uvo\u013e\u0148ovanie enz\u00fdmov po\u0161kodzuj\u00facich tkanivo, ako aj nekontrolovate\u013en\u00fa produkciu ROS. Common complications of poorly compensated diabetes include diabetic foot. The contribution of neuropathy and vasculopathy to the development of diabetic foot is well described. However, current research points to the fact that diabetes mellitus enters into the regulation of healing processes, affecting vascular properties and functionality, resulting in decreased NO bioavailability and increased ROS generation. Disturbed production of growth factors affects the composition of the extracellular matrix, which slows down repair and regeneration processes.
